Analysis of RV components after reoperation of the right ventricular outflow tract in patients with Tetralogy of Fallot

چکیده
Methods 31 patients (20.6 ± 10.3 years) underwent an MRI examination before and after (20 ± 18 months) pulmonary valve replacement or reconstruction (PVR) for the volume overload of the RV. For segmental analysis of the RV, a custom made analysis software was developed. The method provides an automatic contour detection algorithm for the determination of the RV blood volume and enables the user to segregate the blood volume of the RVOT and the corpus based on anatomic landmarks and to generate volume-time-curves over a cardiac cycle. The analysis of volume-time-curves of the RV allowed calculation of Peak-Filling-Rate (PFR) and Time-to-Peak-Filling-Rate (TPFR), which are surrogate parameters for diastolic dysfunction.

Main indications and long-term outcomes of reoperation after initial repair of tetralogy of Fallot
Background and Objective: The aim of this study was to analyze our indications, surgical procedures, and clinical outcomes of patients undergoing reoperation after surgical correction of tetralogy of Fallot (TOF). Methods: Thirty seven consecutive patients who underwent reoperation late after intra-cardiac repair of TOF within a period of 10 years were assessed. Results: The most co...
متن کاملSurgical Management of Chronic Pulmonary Regurgitation After Relief of Right Ventricular Outflow Tract Obstruction
Relief of right ventricular (RV) outflow tract obstruction in tetralogy of Fallot or similar physiology often results in pulmonary regurgitation (PR). The resultant chronic volume overload can lead to RV dilatation, biventricular dysfunction, heart failure symptoms, arrhythmias and sudden death. Although pulmonary valve replacement (PVR) can lead to improvement in the functional class and a sub...
متن کاملAlterations in regional right ventricular shape in patients following tetralogy of Fallot repair involving a transannular Patch
Background Patients who undergo surgical repair of Tetralogy of Fallot (TOF) often experience abnormal remodeling of the right ventricle (RV), notably with chamber dilatation and associated dysfunction. The ideal surgical technique is controversial, but RV outflow tract repair utilizing a transannular patch (TAP) is employed to overcome the RV outflow tract obstruction in many patients with TOF...
متن کاملRight ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...
متن کاملResidual problems with repaired tetralogy of fallot.
Operable Problems Pulmonary regurgitation and RVOT stenosis are anatomical complications that can be relieved only by anatomical repair including surgery and percutaneous catheter treatment.4,5 Therefore, appropriate timing of such intervention is important. However, definite criteria for reoperation are not yet determined. There were several advocated criteria for re-intervention using magneti...
متن کاملFunctional analysis of the anatomical right ventricular components: should assessment of right ventricular function after repair of tetralogy of Fallot be refined?
OBJECTIVE Follow-up after tetralogy of Fallot (ToF) repair is directed to detect timely right ventricular (RV) dysfunction by following pulmonary regurgitation and global RV size, with little attention for the effective contribution of regional RV dysfunction. This study investigates the contribution of regional RV dysfunction on exercise capacity after ToF repair. METHODS Forty-two patients ...
